Internationally trained internal medicine specialists can obtain SCFHS specialist classification through the Prometric-based Specialist Examination. This is a single 150-MCQ computer-based test. For those entering the Saudi Board residency pathway, certification involves a Part 1 exam followed by final written and OSCE assessments. The Saudi Board Internal Medicine residency lasts 4 years.

Internationally trained specialists follow Route A: the SCFHS Specialist Prometric Exam. Candidates receive 3 regular attempts. If all 3 are failed, eligibility is locked. A 4th attempt requires a formal appeal through Mumaris Plus with proof of remedial measures such as CME credits or supervised clinical attachments. If the 4th attempt is also failed, a mandatory waiting period or full retraining applies.
For Saudi Board residents, the pathway includes a Part 1 exam (PGY-1/2), a final written exam, and a final OSCE.
Purpose: Assesses comprehensive knowledge in general internal medicine, subspecialties, and acute care.
Format: 150 MCQs, single best answer.
Duration: 3 hours (180 minutes).
Passing Standard: Passing is determined using SCFHS standard-setting procedures; scores commonly fall around 60–65%.
